ENROLLMENT REQUIREMENTS

BRING PHOTOCOPIES—NO COPIES WILL BE MADE

All participants must provide the following with their registration packet with NO EXCEPTIONS.

Copy of:

• Current immunization record
• Current physical examination information (to be completed on the NYSP Medical Form found on this site) with signed signature of attending physician
• Copy of birth certificate (FIRST TIME PARTICIPANTS ONLY)
• The completion and submittal of all other Registration Forms found on the Enrollment Requirements Page
  

AGE ELIGIBILITY

In order to be eligible for our NYSP Program, children must be between the ages of 10 and 16. A child can be registered at nine years old as long as they will turn 10 years old by August 31st of the program year. A child who is 17 years old may be registered if they have not turned 17 by May 31

  REGISTRATION DATES

There will be only one registration date:

Thursday, April 10, 2008 from 6:00 pm to 7:30 pm

You may register your child up to May 2, 2008. To do so you must bring your forms to the office during regular business hours.

REMINDER:

YOU MAY ONLY REGISTER CHILDREN YOU HAVE LEGAL GUARDIANSHIP FOR.
